How To make an Appointment
To make an appointment, you can use our online booking system on this page. You can also download our booking app for your smart phone or tablet. Click on the links below to download. Appointments can also be made by calling reception on 8162 9817.
All consultations and procedures are on an appointment basis. Emergency cases – whether illness or accident will be seen as promptly as possible. Reception staff must be notified or urgency. Appointments are made at 10-15 minute intervals. Longer consultation or procedure times are available if required. Please notify the receptionist when making your appointment. If more than one family member is to be seen by the doctor, please advise the receptionist at time of booking so an additional appointment can be booked. Emergency or longer consultations could delay your appointment. For continuity of care, patients are encouraged to make appointments with the usual GP whenever available.

HOME VISITS
We ask patients to make every effort to attend the surgery for consultations, however we will visit our regular patients at home wherever possible if their condition prevents them from coming to the surgery and if they live within a reasonable distance to the surgery.

While Doctors and Nurses treat emergency situations that may sometimes occur during a general appointment, the practice is NOT an emergency clinic. Hampstead Health does not have all of the necessary life saving medical equipment that hospital emergency rooms have.

Opening Hours
Hampstead Health Family Practice now has extended opening hours and continue to bulk bill during these times. Opening hours are 8.30am-11pm Monday to Friday, and 8am-5pm Saturdays, and 9am-5pm Sundays.
We open some public holidays, so please keep informed via our website, Facebook Page, or in-house signage.

Changes to our billing policy
From 1st December 2022 there will be a $20 gap payment for face to face as well as telehealth consultations for patients with a Medicare card
Patients 15 years old or younger, healthcare, pension and DVA card holders, will continue to be bulk billed weekdays between 8am to 6pm Monday through to Friday. All other times the $20 gap is payable
Patients returning for test results (must be within 2 weeks and with their regular GP or the GP who requested the tests), consultations for chronic disease and mental health care plans, health assessments, home medicine reviews and flu clinics, telephone consults for Covid positive or patients awaiting PCR results will continue to be bulk billed all hours
Please note, payment for the gap fee as well as the Medicare item fee is payable up front at the time of consultation Payments are accepted by credit card, debit card or cash (surcharges apply for card payments)
*Telephone consultations must be paid prior to the appointment and will incur a surcharge which is approximately $1.37 for the transaction on top of the $20 gap fee

Why SMS?
As well as being a more environmentally-friendly alternative to traditional letters, SMS is more secure because you can only read the health message after verifying your personal information, and more reliable because messages have less chance of being lost.
What do I need to do?
When you receive an SMS, open the message and click on the secure link. You’ll be taken to a screen where you will need to verify your personal information, including your last name and date of birth. Once you have verified your identity you can read your recall health message. From here you can book your appointment for a time convenient to you.
Please note that you’ll need to be connected to the Internet, or have data on your phone, for the link to work. If your phone is connected to the internet and you still can’t access the link, please call us at 08 8162 9817.

Prescription Renewals
All patients are required to have an appointment to renew prescriptions.

We request that you advise us a minimum of 2 hours before your scheduled appointment if you are unable to make it.

Communication Via E-Mail Policy

Introduction
This privacy policy is to provide information to you, our patient, on how your personal information (which includes your health information) is collected and used within our practice, and the circumstances in which we may share it with third parties.
Why and when your consent is necessary
When you register as a patient of our practice, you provide consent for our GPs and practice staff to access and use your personal information so they can provide you with the best possible healthcare. Only staff who need to see your personal information will have access to it. If we need to use your information for anything else, we will seek additional consent from you to do this.
Why do we collect, use, hold and share your personal information?
Our practice will need to collect your personal information to provide healthcare services to you. Our main purpose for collecting, using, holding and sharing your personal information is to manage your health. We also use it for directly related business activities, such as financial claims and payments, practice audits and accreditation, and business processes (eg staff training).
What personal information do we collect?
The information we will collect about you includes your:
- names, date of birth, addresses, contact details
- medical information including medical history, medications, allergies, adverse events, immunisations, social history, family history and risk factors
- Medicare number (where available) for identification and claiming purposes
- healthcare identifiers
- health fund details.
Dealing with us anonymously
You have the right to deal with us anonymously or under a pseudonym unless it is impracticable for us to do so or unless we are required or authorised by law to only deal with identified individuals.
How do we collect your personal information?
Our practice may collect your personal information in several different ways.
- When you make your first appointment our practice staff will collect your personal and demographic information via your registration.
- During the course of providing medical services, we may collect further personal information.
- We may also collect your personal information when you send us an email or SMS, telephone us, make an online appointment or communicate with us using social media.
- In some circumstances personal information may also be collected from other sources. Often this is because it is not practical or reasonable to collect it from you directly. This may include information from:
- your guardian or responsible person
- other involved healthcare providers, such as specialists, allied health professionals, hospitals, community health services and pathology and diagnostic imaging services
- your health fund, Medicare, or the Department of Veterans’ Affairs (as necessary).
When, why and with whom do we share your personal information?
We sometimes share your personal information:
- with third parties who work with our practice for business purposes, such as accreditation agencies or information technology providers – these third parties are required to comply with APPs and this policy
- with other healthcare providers
- when it is required or authorised by law (eg court subpoenas)
- when it is necessary to lessen or prevent a serious threat to a patient’s life, health or safety or public health or safety, or it is impractical to obtain the patient’s consent
- to assist in locating a missing person
- to establish, exercise or defend an equitable claim
- for the purpose of confidential dispute resolution process
- when there is a statutory requirement to share certain personal information (eg some diseases require mandatory notification)
- during the course of providing medical services, through My Health Record (eg via Shared Health Summary, Event Summary).
Only people who need to access your information will be able to do so. Other than in the course of providing medical services or as otherwise described in this policy, our practice will not share personal information with any third party without your consent.
We will not share your personal information with anyone outside Australia (unless under exceptional circumstances that are permitted by law) without your consent.
Our practice will not use your personal information for marketing any of our goods or services directly to you without your express consent. If you do consent, you may opt out of direct marketing at any time by notifying our practice in person or in writing.
How do we store and protect your personal information?
Our practice stores all personal information securely in an electronic format. Personal passwords are used for all staff who have access to clinical records. Staff who may need to access your health information have signed a confidentiality agreement. Access limitations are set in accordance to the staff members role within the practice.
How can you access and correct your personal information at our practice?
You have the right to request access to, and correction of, your personal information.
Our practice acknowledges patients may request access to their medical records. We require you to put this request in writing and our practice will respond within a reasonable time, usually within 30 days. Access to your personal health record will incur a fee to cover administration costs, these costs vary depending on the size and complexity of the request. Our practice will take reasonable steps to correct your personal information where the information is not accurate or up to date. From time to time, we will ask you to verify that your personal information held by our practice is correct and current. You may also request that we correct or update your information, and you should make such requests in writing to the practice.
How can you lodge a privacy-related complaint, and how will the complaint be handled at our practice?
We take complaints and concerns regarding privacy seriously. You should express any privacy concerns you may have in writing to the Practice manager, Hampstead Health Family Practice, Level 1, 237 Hampstead Road, Lightsview, SA, 5085. We will then attempt to resolve it in accordance with our resolution procedure usually within 30 days.

This privacy policy will be reviewed regularly to ensure it is in accordance with any changes that may occur. Any changes to our policy will be published on the practice website and made available at the practice.

It is the policy of this practice to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff. We listened to you and we are improving patient care
At Hampstead Health Family Practice we carried out a patient survey (the Practice Accreditation and Improvement Survey) and asked for your honest opinions on the service we provide. We listened to your concerns and have taken the following actions in order to make improvements to patient care where you feel it would be most useful:

We understand these questions are annoying, particularly when you have been a regular patient of the practice however these identification questions, address and contact details must be checked for every patient as you arrive. Details such as address and contact details change frequently therefore we need to ensure we have these details correct at each visit. This information is used in any referrals we may provide to you and is also critical for any follow up of test results Additionally, there are many patients with the same or similar name so our staff need to check we have the correct patient at the time of checking in. | The implementation of our security/COVID screens has made talking to you and confirming your details for patient identification difficult. We have now installed microphones for patient’s piece of mind, this elevates them having to communication their private/confidential information loudly. These microphones allow both reception staff and patients to talk softly into the microphone which is then clearly heard by staff and our patients alike.
